R12 FND_CANT_INSERT_USER_ROLE

Have the following issue in my R12 instance. Trying to add “Functional Administrator” (or) “Functional Developer” responsibility to SYSADMIN gives the attached error.

–> No issues adding these responsibilities to a different/new user
–> Looks like this issue happens after RUP2
Would be investigating this issue further/log an SR with Oracle.
